It is remarkable that SSS has recently made a prominent leap by making its service accessible to its valued members.

It can't be denied that the availability of the SSS website has extraordinarily helped Filipino members. With online access, it is currently simpler for members to connect with SSS to ask for data or apply for loans.

Nevertheless, there are still transactions that can't be done online just like filing for an SSS maternity benefit application.

SSS requires all members including the employers to file all maternity benefit applications to the nearest SSS branch. In spite of the fact that SSS has given online access to the majority of its services, this one is an exception.

Furthermore, SSS requires different sets of forms or documents depending upon the member's classification when applying for SSS Maternity Benefit.

Knowing the required documents in advance could save time and effort from returning to the SSS office.

List of Requirements for Maternity Benefit Application


Here are the required documents when applying for the SSS maternity benefits.

1. MBRA Form or MBA Form


If the claimant is employed, then, the form to fill up and submit is the Maternity Benefit Reimbursement Application (MBRA) form.

If the claimant is self-employed or voluntary, then the form to fill up and submit is the Maternity Benefit Application (MBA) form.

2. Maternity notification form


This is the form submitted to SSS prior to delivery for notification of pregnancy. This has to be marked "received" and not a newly filled-up form. 

If maternity notification was submitted through the website or SSIT, then, a copy or printout of the confirmation is to be attached to the application.

3. Proof of pregnancy


Those with normal delivery need to present the baby's birth certificate or fetal death certificate appropriately registered with the LCR. 

Those with cesarean delivery need to present the baby's birth certificate or fetal death certificate appropriately registered with the LCR and any of the following supporting documents issued by the hospital:

1. Operating Room Record
2. Surgical Memorandum
3. Discharge Summary Report
4. Medical/Clinical Abstract
5. Delivery Report
6. A detailed invoice showing cesarean delivery charges if delivery happens abroad.

For those with complete miscarriage, they need to present the obstetrical history demonstrating the quantity of pregnancy certified by the attending physician and any of the following supporting documents:

1. Pregnancy test before and after the incident
2. Ultrasound report showing evidence of pregnancy 
3. Medical Certificate issued by attending physician

Those with incomplete miscarriage need to present the obstetrical history duly signed by the attending physician and any of the following supporting documents:

1. A certified true copy of medical records
2. Dilation and curettage report
3. Histopathological report
4. Pregnancy test before and after the incident
5. Ultrasound report 

Those with ectopic pregnancy need to present the obstetrical history and any of the following documents:

1. A certified true copy of medical records
2. A certified true copy of ORR
3. Histopathological report
4. Pregnancy test before and after the incident

Those with hydatidiform mole need to present the obstetrical history duly signed by the attending doctor, D & C report, and histopathological report.

4. Member or Filer's SSS ID or other valid IDs.


If the application is filed by the member herself, needs to have 1 primary ID or 2 secondary IDs. 

If the application is filed by a representative, 1 primary ID of both the member and the representative is needed. If both do not have any of the primary IDs, then, they could present 2 secondary IDs of both of them.

If the application is filed by the business or household employer, still needs 1 primary or 2 secondary IDs.

If the application is filed by the company representative, then, there is a need to show the ACR card or LOA and the company ID of the representative.

If the application is filed by the employer representative, then, an LOA and the company ID of the representative are needed.

Additional Requirements for SSS Maternity Benefit

There are additional documents you also need to bring when applying for the SSS maternity benefits.

1. Certificate of separation from employment


If delivery, miscarriage, or operation happens during the employment period, after the date of separation, or within 6 months from the date of separation.

Then, a certificate of separation from employment is required to determine the effective date of separation and to confirm that there was no advance payment was granted to the member.

But there is no certificate of separation required for the self-employed or previously employed SSS member if the following circumstances are happening:

- If the company she is/was working with is on strike
- If the company has been dissolved or has stopped operation
- If there is a pending case regarding the separation of an employee
- If the member is separated from employment due to AWOL or has a strained relationship with the employer

2. English-translated and authenticated foreign documents


If delivery or miscarriage happens abroad, then, the documents should have an English translation and should be authenticated by the Philippine Embassy or Consulate Office.
